Covid-19 Vaccination Policy
In the interest of protecting the health and safety of our members, the MVM board has decided that MVM swimmers must be fully vaccinated against COVID in order to attend workouts, with some exceptions noted below. We will institute this requirement starting on October 1, 2021.
To implement this policy, we created a Google form asking for proof of vaccination status via a QR code. Active members can get a link by mailing mvmcontact(at)google.com. Digital proof of vaccination can be created here: https://myvaccinerecord.cdph.ca.gov/. As with personal contact information, vaccination status information will be held in secure and strict confidence.
We will honor verified medical exceptions. If you have a medical exception, please let us know by emailing the MVM board.
We also understand that certain people are vaccinated but are not able to get a digital copy of the vaccine record (e.g., vaccinations received from a federal agency, for example, Department of Defense, Indian Health Services, or Veterans Affairs). If you received the vaccine and do not have digital proof of vaccination status, please let us know by emailing the MVM board.
We are asking everyone to provide proof of vaccination via the Google form or to email the MVM Board if you fall into the exceptions no later than September 30, 2021. Anyone who is not willing to provide proof will be refunded the unused portion of their quarterly or half-year dues. Unfortunately, without proof of vaccination, you will not be able to swim workouts with MVM beginning October 1, 2021.
Also, because of this new policy, we will no longer be allowing guests to swim workouts until further notice.
